Estimated Unit Duration: 6 hours In-Person
Participants learn about the case management steps necessary to support youth through age 19 who are wards of the state in their preparation for adult life. Topics include: identifying a Transition Team; transition planning with youth and Transition Team; developing and managing a youth-driven Transitional Living Plan (TLP); maintaining records; evaluating and tracking progress; identifying services and resources in the areas of education, employment, health care, finances, housing, relationships, and adult services; monitoring progress; providing key documents; and closing the case.
The minimum number of participants needed for this unit is 6, maximum is 15. Consult your supervisor if you are interested in taking this unit.
